This daily home monitoring equipment is a small machine placed in the patient's home that callects an individuals' vital signs such as weitgh, blood pressumre, O2 sat level, heart rate and temperature. Each day a calming voice guides the patient through a simple procedure to collect this data and then sends this information to nurses at Susquehanna Home Care for evaluation. Nurses notify the patient's physician if there are any changes in condition so problems may be addressed erarly - preventing an emergency room or hospital admission
